Thank you for selecting Mehan Eye for your eye care. We are committed to providing quality eye care. Under the Patient Forms page you will find a patient profile form and a medical history form to complete prior to your appointment. Bring those forms with you to the appointment. Also bring the following:

1. A current list of all of your medication and the dosages.
2. Any and all insurance cards so that we may make copies and correctly bill your insurance.
3. A photo ID.
4. A referral if your insurance requires it

Your first visit at Mehan Eye will consist of a complete eye exam. The initial visit usually lasts from 1 to 1.5 hours, but may vary in length depending on the diagnosis made by your doctor. Your eyes may be dilated during this visit and we recommend you bring a pair of sunglasses to protect your eyes from the sun. You may wish to have someone accompany you.
